Six people ran for three open positions on the board. One of the winners later declined, before she was inducted at the first meeting. My understanding is that this is not a vacancy (she did not actually serve). Can we go to the net highest runner-up from the ballot? Or do we need to hold a new election?

How you handle this depends on when the member elected takes office and what your bylaws state about filling vacancies. If the bylaws state that the person takes office immediately after the adjournment of the meeting, you have a vacancy. If the bylaws state the person takes office at the induction then you need to have another election. Look at your bylaws carefully.
